If you've already scheduled a day and time for your intro lesson, then in order to confirm and lock down the slot that you've selected you'll need to complete and submit this "New Student Info" form (below) which will help me provide you with the best possible lessons experience. If you haven't contacted me yet about selecting a day and time for the free intro lesson, no worries at all; as soon as I can once I receive your completed form, I'll reach out to you to get everything all set up for that.

Once you've completed all the required fields and clicked the "Submit" button in the form window, and you're able to see the "Thank You" page in the form viewer, please just then click "Finished" below the form viewer and you'll be returned to my main website. Here is the form viewer:
